Five ways to put benefits on the table

Most small businesses end up with a mix: one or two of these carried by the business, the rest offered to employees at their own cost. Here is what each one actually does, and what it does not.

Group major medical

employer-sponsored health coverage for your team: the business picks the plan design and how much of the premium it pays, employees enroll (family members too, where offered), and the owner can sit on the same plan. Groups can start at 2 employees.
it is not quoted online. Group pricing depends on your team's census and state, so anything you see before that is the general picture, not your number.

Group dental & vision

the benefits employees actually use every year (cleanings, fillings, glasses and exams) offered as a group plan the business can fund, share, or simply make available. Often the easiest first benefit to add.
it is not medical coverage, and waiting periods on major dental work can apply depending on group size and prior coverage. We spell those out before anyone enrolls.

ICHRA / defined contribution

the business sets a fixed monthly allowance per employee; each employee picks their own individual health plan and is reimbursed up to that amount. The owner controls the budget; employees control the plan. Reimbursements are generally tax-advantaged where the rules allow.
it is not one group plan everyone shares: employees shop individually, and it has setup and documentation rules that vary; an advisor confirms whether it fits your team before you commit.

Voluntary / worksite benefits

accident, hospital-cash, critical-illness and life coverage employees choose and pay for themselves through payroll, often at group rates they could not get on their own. The business offers it; the business does not fund it.
it is not health insurance and does not replace a medical plan. It pays cash on specific events so a bad week does not become a missed rent check.

Group life & disability

a base amount of life insurance for every employee, and income protection if someone cannot work because of illness or injury, usually simple to enroll, often with no medical questions up to a set amount for eligible groups.
base group life is a foundation, not a full family plan, and disability benefits replace part of pay for a defined period. Employees who need more can add individual coverage on top.
