The short answer: a PPO gives you the most freedom to choose doctors. An EPO trades that freedom for a usually lower price. An HSA-eligible plan pairs a higher deductible with a tax-advantaged savings account. If there is a doctor you will not give up, start there: check the network first, then pick the type.
PPO: the most choice
A PPO lets you see out-of-network doctors, at a higher cost. Many private plans are nationwide PPOs: you can see doctors across state lines, with no referrals. It suits people who travel, split time between states, or have specialists they want to keep.
EPO: in-network only, usually cheaper
An EPO covers in-network care only, unless it is an emergency, which is treated as in-network. It is usually cheaper than a PPO. It suits people whose doctors are all in the network and who would rather pay less each month.
HSA-eligible: higher deductible, plus a savings account
An HSA-eligible plan pairs a higher deductible with a tax-advantaged savings account. How the account works for you depends on your own situation, so for the tax side, talk to your tax professional. We help you weigh the deductible against what you would pay each month.
And the Marketplace?
Many Marketplace plans are HMO or EPO, with networks local to your county. That can work well if your doctors are local and in-network. If they are not, or you want coverage that travels, a private PPO may fit better. See ACA and private plans side by side.
Picture it
You picked the cheapest plan on the list last year without looking at the letters. In March your knee needs a specialist, and the one your friend swears by is out of network. On an EPO, that visit is not covered. You start over with a name from the directory.
Now picture the other version. Before you enroll, you tell us the doctors you want to keep. We check them by name, and only then compare plan types. Maybe the EPO still wins because everyone is in-network. Maybe the PPO is worth the difference. Either way, you knew before March.
The one question to answer first
- Is there a doctor you will not give up? Then the network matters more than the letters. Networks change, so we verify at enrollment, not from memory.
- Do you travel or live in more than one state? Look at nationwide PPOs.
- Healthy, with doctors in-network? An EPO or an HSA-eligible plan may cost less each month.
Every plan card in our Plan Finder links straight to that network's directory, so you can look up a doctor yourself. Starting a plan late in the year? Check when its deductible resets.
This is general education, not tax advice and not a guarantee of coverage. Networks, pricing and eligibility depend on your state, your location and underwriting.
